Глаукома является одной из ведущих причин слепоты и инвалидности по зрению. В Российской Федерации на 01.01.2022 г. зарегистрировано около 1,2 млн человек с глаукомой, из них 101 тыс.– инвалиды. Рост заболеваемости глаукомой связан с увеличением доли пожилого населения, для которой характерна высокая коморбидность. В связи с этим актуальным становится поиск новых методов профилактики слепоты и лечения глаукомы. Цель: выявить связь между применением статинов и возникновением или прогрессированием глаукомы. Материалы и методы. Проведен систематический поиск научных публикаций в электронных базах данных PubMed, Google Scholar, Web of Science, Scopus и eLibrary за период с 2000 по 2025 г. Поиск осуществлялся по ключевым словам «глаукома», «статины», «холестерин», «открытоугольная глаукома». Критерии включения: оригинальные исследования, метаанализы, систематические обзоры, посвященные изучению связи между приемом статинов и глаукомой. В итоге было отобрано 38 работ, среди которых 25 оригинальных исследований. Результаты. Большинство проанализированных публикаций указывают на потенциальный протективный эффект статинов при глаукоме. Установлено, что длительный прием статинов (более 2–3 лет) коррелирует со снижением риска развития первичной открытоугольной глаукомы на 21–40 %. Предполагается несколько механизмов влияния статинов: во-первых, нейропротекторное действие на ганглиозные клетки сетчатки за счет улучшения микроциркуляции, снижения апоптоза и цитотоксичности; во-вторых, модуляция иммунных реакций; в-третьих, потенциальное влияние на внутриглазное давление за счет улучшения оттока внутриглазной жидкости. Заключение. Проведенный литературный обзор демонстрирует сложную и неоднозначную связь между приемом статинов и развитием первичной открытоугольной глаукомы. Несмотря на преимущественно положительные результаты, существуют противоречивые данные. Необходимы масштабные рандомизированные клинические исследования для окончательного подтверждения нейропротекторных свойств статинов при глаукоме. Представленные данные открывают перспективы для разработки новых терапевтических стратегий лечения глаукомы с использованием статинов.</p></abstract><trans-abstract xml:lang="en"><p>Background. Glaucoma is one of the leading causes of blindness and visual impairment worldwide. As of January 1, 2022, approximately 1.2 million people with glaucoma were registered in the Russian Federation, including 101 000 individuals with visual disability. The rising prevalence of glaucoma is associated with population aging and a high burden of comorbidity in older adults. This makes the search for new approaches to preventing glaucoma-related blindness and improving glaucoma treatment particularly relevant. Purpose: To investigate the association between statin use and the development or progression of glaucoma. Materials and methods. A systematic literature search was conducted in PubMed, Google Scholar, Web of Science, Scopus, and eLibrary for publications from 2000 to 2025. The search terms were glaucoma, statins, cholesterol, and primary openangle glaucoma. Eligible publications included original studies, meta-analyses, and systematic reviews examining the association between statin use and glaucoma. Overall, 38 publications were included, of which 25 were original studies. Results. Most of the studies reviewed suggest a potential protective effect of statins in glaucoma. Long-term statin use, defi ned as treatment for more than 2–3 years, was associated with a 21–40 % lower risk of developing primary open-angle glaucoma. Several mechanisms have been proposed, including neuroprotection of retinal ganglion cells through improved microcirculation and reduced apoptosis and cytotoxicity; modulation of immune responses; and a potential reduction in intraocular pressure through improved aqueous humor outfl ow. Conclusion. This literature review indicates a complex and heterogeneous association between statin use and open-angle glaucoma development. Although most findings suggest a potential benefit, the evidence remains inconsistent. Large randomized clinical trials are needed to confirm the neuroprotective effects of statins in glaucoma.
