Термин «синдром дисфункции хрусталика» (Dysfunctional Lens Syndrome, DLS) описывает возрастные изменения хрусталика, включая начальную катаракту и пресбиопию, которые не вызывают выраженного снижения остроты зрения. Этот синдром сопровождается уменьшением объема аккомодации, увеличением светорассеяния, снижением контрастной чувствительности. С увеличением продолжительности жизни распространенность DLS растет – и возникает необходимость в более глубоком изучении патогенетических механизмов, лежащих в его основе, и детализации диагностических критериев для формирования стандартов коррекции и лечения. Цель: обобщение информации о патогенезе DLS, возможностях его стадирования с помощью объективных методов обследования, рекомендациях по лечению. Материалы и методы. Проведено библиографическое исследование научных публикаций баз данных информации PubMed, ScienceDirect, CyberLeninka. Поиск литературных источников проведен по следующим ключевым словам: синдром дисфункции хрусталика, коррекция пресбиопии, катаракта, возрастные изменения хрусталика. Из более чем 100 просмотренных источников в работу вошли 32 публикации, в основном за последние 10 лет. Результаты. Синдром DLS включает в себя широкий спектр состояний, начиная от ранних стадий, характеризующихся потерей амплитуды аккомодации и незначительными аномалиями рефракции, до поздних, когда возникает снижение остроты и качества зрения за счет увеличения светорассеяния и аберраций. В настоящее время для оценки возрастных изменений хрусталика проводят исследование остроты зрения и определение степени помутнения хрусталика при осмотре за щелевой лампой. Кроме того, возможно использовать объективные инструментальные методы оценки степени помутнения хрусталика с использованием оптической когерентной томографии и Шаймпфлюг-камеры. Для коррекции и лечения синдрома DLS используются различные способы: от очков и контактных линз до хирургических вмешательств, таких как удаление хрусталика и имплантация интраокулярной линзы. Исследуются также препараты, направленные на замедление прогрессирования изменений хрусталика. Заключение. Термин «синдром дисфункции хрусталика» в настоящее время применяется для описания начальной катаракты и пресбиопии, когда снижение остроты зрения еще незначительно и при этом у пациента присутствуют жалобы на зрение из-за возрастных изменений хрусталика. Для разработки наиболее эффективного и достаточного подхода к коррекции DLS и, соответственно, устранения возникающих жалоб необходимы дальнейшие исследования, направленные на разработку и стандартизацию диагностических критериев, а также оценку эффективности новых методов и средств лечения.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. The term “Dysfunctional Lens Syndrome” (DLS) refers to age-related changes in the lens, including early cataracts and presbyopia, that do not significantly reduce visual acuity. This syndrome is characterized by a decrease in accommodative amplitude, an increase in light scattering, and a reduction in contrast sensitivity. With rising life expectancy, the prevalence of DLS is increasing, necessitating a more in-depth study of the pathogenic mechanisms underlying the syndrome and the refinement of its diagnostic criteria to establish standards for treatment correction. Aim: to summarize information on the pathogenesis of DLS, the potential for staging the syndrome using objective examination methods, and to provide treatment recommendations. Materials and methods. A bibliographic study of scientific publications was conducted using the Pubmed, ScienceDirect, and Cyberleninka databases. Literature sources were searched using the following keywords: dysfunctional lens syndrome (DLS), presbyopia correction, cataract, age-related lens changes. A total of 32 publications, primarily from the last 10 years, were included in the study from more than 100 viewed sources. Results. The DLS encompasses a wide range of conditions, from early stages characterized by a loss of accommodative amplitude and minor refractive anomalies to later stages involving a decrease in visual acuity and quality due to increased light scattering and aberrations. Currently, the evaluation of age-related lens changes involves assessing visual acuity and determining the degree of lens opacification using slit-lamp examination. Additionally, objective instrumental methods, such as optical coherence tomography and Scheimpflug imaging, can be used to assess the degree of lens opacification. Various methods are employed for the correction and treatment of DLS, ranging from glasses and contact lenses to surgical interventions, such as lens extraction and intraocular lens implantation. Research is also ongoing into drugs aimed at slowing the progression of lens changes. Conclusion. The term “Dysfunctional Lens Syndrome” is currently used to describe early cataracts and presbyopia, where the reduction in visual acuity is still minor, but patients experience visual complaints due to age-related lens changes.
