Кератоконус – дегенеративная патология роговицы, которая приводит к ее истончению с последующим изменением кривизны и преломляющей способности, что может снижать остроту зрения до светоощущения. Трансплантация роговицы является методом лечения на поздней стадии заболевания и относится к операции с благоприятным прогнозом прозрачного приживления трансплантата. Цель: анализ клинического случая сквозной кератопластики у пациента с диагнозом «кератоконус на фоне сопутствующего синдрома Сторморкена» – редкого генетического заболевания, связанного с мутацией гена молекулы взаимодействия стромы 1 (stromal interaction molecule 1 – STIM1). Описание случая. Пациент З., мужчина 24 лет с поставленным по клиническим данным в детстве диагнозом «синдром Сторморкена», попал на консультацию в Московский городской офтальмологический центр (МГОЦ) ГБУЗ «Московский многопрофильный научно-клинический центр им. С.П. Боткина». После сбора жалоб, анамнеза, данных офтальмологического исследования (биомикроскопии, кератопахиметрии и ультразвукового исследования глаза) был установлен диагноз «кератоконус обоих глаз». Позднее, ввиду прогрессирования процесса, пациенту была проведена сквозная кератопластика правого и левого глаза. После проведенной двусторонней трансплантации роговицы острота зрения правого глаза = 0,8 н/к; левого глаза = 0,7 н/к. При ведении пациента обращали на себя внимание следующие клинические признаки: двусторонний миоз (рефрактерный к медикаментозным препаратам), гипоспления, тромбоцитопения, дислексия, проксимальная мышечная слабость. При сопоставлении с критериями, указанными в зарубежной литературе (по описанию других случаев), данный симптомокомплекс соотносился с синдромом Сторморкена. После выписки пациенту была проведена консультация врача-генетика и молекулярно-генетическое исследование, подтвердившее патогенную мутацию в гене STMI1. Заключение. В данном клиническом случае впервые, по данным литературы, представлен пациент с двусторонним кератоконусом и сопутствующим синдромом Сторморкена, который успешно перенес сквозную кератопластику обоих глаз. Более того, анатомические особенности переднего отдела глаза обеспечили высокую остроту зрения в раннем послеоперационном периоде.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Keratoconus is a progressive corneal disease that results in thinning, curvature change, and refractive alterations, significantly impairing. visual acuity, even to the point of light perception. In advance stages, corneal transplantation may be necessary. Purpose. This publication aims to analyze a clinical case of corneal transplantation in a patient with keratoconus and Stormorken Syndrome, a rare genetic disorder caused by mutations in the STIM1 gene leading to calcium channelopathies. Case description. Patient Z., a 24-year-old male diagnosed with Stormorken Syndrome in childhood, was referred to the Moscow City Ophthalmological Centre. Comprehensive evaluations, including biomicroscopy, keratopachimetry, and ultrasound eye examination, confirmed keratoconus in both eyes. The patient underwent bilaterial penetrating keratoplasty due to disease progression. Postoperatively, visual acuity was 0.8 in the right eye, and 0.7 in the left eye. Additional clinical observations including bilateral myositis (resistant to medication), hypoinflammation, thrombocytopenia, dyslexia, and proximal muscles weakness, of the consistent with Stormorken Syndrome. Genetical testing confirmed a pathogenic STMI1 mutation. Conclusion. Despite the high volume of keratoplasty procedures at our clinic, this case is noteworthy.
